Welding is often a critical point in determining product quality, where various types of defects can significantly impact the final result. The research was conducted using a descriptive qualitative approach based on inspection data collected over 16 weeks, and data analysis was carried out through the DMAIC stages (Define, Measure, Analyze, Improve, and Control). The analysis revealed that the most dominant defects were porosity, cluster porosity, spatter, and undercut, with the main root causes being human and method factors. A five-week simulation of improvement showed a significant reduction in DPMO, from 26,500 to 4,900, and an increase in sigma level from 3.12 to 4.03. Based on the <em>fishbone</em> and pareto diagrams, several defect reduction strategies were formulated, including retraining of welding operators, strengthening SOP implementation, enhancing visual supervision, and conducting regular maintenance of welding machines. These conditions result in prolonged patient waiting <br>times, delayed medical information flow, and suboptimal utilization of healthcare resources. This study <br>aims to identify hospital information system requirements, analyze current service-related problems, <br>and design an integrated Hospital Management Information System (HMIS) combined with an <br>Electronic Medical Record (EMR) framework as an Industry 4.0–based solution. This research adopts <br>a descriptive–analytical approach grounded in industrial engineering principles. Data were collected <br>through an extensive literature review, conceptual observation, interviews, time–motion studies, and <br>user requirement simulations conducted in a Type C hospital setting. System development was carried <br>out using Data Flow Diagrams (DFD), Entity Relationship Diagrams (ERD), and business process <br>flowcharts. System performance was evaluated through simulation modeling using Arena software, <br>focusing on key operational efficiency indicators, namely cycle time and system utilization. The <br>simulation results demonstrate that the proposed integrated HMIS–EMR design significantly improves <br>operational performance, evidenced by a reduction in service cycle time from 120 minutes to 45 <br>minutes and a decrease in system utilization from 92% to 78%. The novelty of this study lies in its <br>holistic approach, which integrates healthcare information system design with operational efficiency <br>analysis and simulation-based validation. The selection of this topic is motivated by the importance of efficient inventory management in the café industry to maintain operational smoothness and reduce costs. The focus of this study is on four main raw materials: Thai Tea, Sweetened Condensed Milk (SCM), Palm Sugar, and Ice Crystals. The research method used is a descriptive-verificative quantitative approach, utilizing historical data on raw material purchases and usage from 2023-2024, obtained through observation, interviews, and document reviews. The research results indicate that the application of the EOQ method can optimise order quantities and procurement frequencies, thereby improving cost efficiency. For example, for Thai Tea, the optimal EOQ is 527 grams with an order frequency of 38 times per year, resulting in total inventory costs of Rp 949,300. Meanwhile, for Palm Sugar, the optimal EOQ reaches 6,347 ml with a Total Cost of Rp 11,432,300. This study also identified the dominance of Safety Stock costs that need to be optimised, particularly for raw materials such as Ice Crystals and Palm Sugar. MI Cilegon. This study aims to analyze the potential hazards in decanter overhaul work activities using the HIRARC (Hazard Identification, Risk Assessment, and Risk Control) method. The study was conducted through direct observation, interviews with employees, and data collection on work accidents and work activities in 2023. Based on the results of the analysis of 8 main work activities, 37 potential hazards were found which were classified into 4 risk level categories: low, medium, high, and extreme risk. Most of the potential hazards are controlled by administrative controls and the use of personal protective equipment (PPE). Raffa Helical Bronze<em>, with a primary focus on reducing spindle component failures. The RCM method is used to identify key equipment functions, possible failure modes, and develop effective maintenance strategies. Data were collected through direct observation, technician interviews, internal documentation, and historical failure analysis. The results showed that the spindle is the critical component that most often experiences damage in the form of overheating, rough noise, and high vibration. Through the application of </em>FMEA<em>, the highest Risk Priority Number (RPN) value was obtained for spindle overheat damage, which was 280. With the implementation of periodic preventive maintenance such as bearing lubrication, coolant checking, and calibration, the </em>MTBF<em> value increased from 143 hours to ±250 hours, and </em>MTTR<em> decreased from 7 hours to ±3 hours. The application of RCM has proven effective in reducing the frequency of spindle failures and increasing the efficiency and efficiency of spindle machines. Understanding the rules for working at heights is crucial because any negligence can lead to fatal accidents, resulting in serious injuries or even death. The number of accidents in Indonesia due to falls from heights remains relatively high. Personal Protective Equipment (PPE) is a device capable of protecting a person by isolating part or all of the body from potential hazards in the workplace. The company has implemented laws and regulations to prevent workplace accidents. This study aims to identify potential hazards in roof repair work in office buildings and implement control measures. This study uses a descriptive analytical method by conducting field surveys and interviews with workers. Data from the field surveys are collected and used to determine control measures to reduce risks and prevent accidents. Personal protective equipment (PPE) is required before carrying out roof repair work in office buildings. The personal protective equipment used includes safety shoes, safety helmets, safety body harnesses, leather gloves, and goggles. In addition to using personal protective equipment, worker competency and supporting equipment must be prepared. The results of the analysis of the use of personal protective equipment in roof repair work in office buildings are to determine control measures that can be used as work procedures in PT.
