This study aims to design and develop a Flow Bottle product based on consumer needs using the Quality Function Deployment (QFD) method. The research employed a quantitative descriptive approach, with data collected through questionnaires distributed to 17 respondents. The research stages included identifying the Voice of Customer (VoC), determining Customer Requirements, establishing Functional Requirements, and constructing a House of Quality (HOQ) to translate customer needs into technical product specifications. The results indicate that the most important consumer requirements are strong bottle material and leak-proof performance, each receiving an importance level of 94.1%, followed by ease of carrying (88.2%), affordable price (70.6%), and attractive design (58.8%). The HOQ analysis reveals that the primary priorities for product development are improving material quality and enhancing the leak-proof system, as these factors have the strongest relationship with customer requirements. Furthermore, the Customer Competitive Assessment shows that the Flow Bottle has competitive advantages in terms of affordability and leak resistance compared to competing products. One risk control effort is the use of personal protective equipment (PPE). Therefore, a job safety analysis (JSA) method is needed to identify hazards in a structured manner at each welding stage to determine the need for appropriate PPE. These potential hazards can trigger work accidents and occupational diseases such as eye injuries and respiratory disorders. This study aims to identify potential hazards at each stage of welding work using the JSA method. This study is a qualitative descriptive study with a JSA approach. Data collection was carried out through direct observation such as material preparation, pipe adjustment, welding process, and interviews with workers or welders. The analysis was carried out in several steps, namely identifying potential hazards at each step, and determining PPE recommendations that meet standards. Based on the JSA worksheet, several potential hazards include burns due to sparks during the welding stage and eye injuries due to pipe debris during the finishing stage. The results of the analysis show a gap between the PPE recommended by the JSA and that actually used in the field. Workers often neglect the use of PPE, such as respirators and leather aprons, due to discomfort and limited inventory in the workshop. The JSA method can map specific hazards and establish appropriate PPE requirements for each welding stage. MERAK CHEMICAL INDONESIA) https://ejurnal.unival-cilegon.ac.id/index.php/sisprotek/article/view/618 <p><em>Occupational safety and health (OHS) is an important aspect in the industrial world, especially in the chemical sector such as PT. Merak Chemical Indonesia. This study aims to analyze the potential hazards in decanter overhaul work activities using the HIRARC (Hazard Identification, Risk Assessment, and Risk Control) method. The study was conducted through direct observation, interviews with employees, and data collection on work accidents and work activities in 2023. Based on the results of the analysis of 8 main work activities, 37 potential hazards were found which were classified into 4 risk level categories: low, medium, high, and extreme risk. Most of the potential hazards are controlled by administrative controls and the use of personal protective equipment (PPE). This study concludes that the application of the HIRARC method is effective in identifying and controlling potential hazards in the decanter overhaul process, as well as providing recommendations for improving occupational safety in the company. </em></p> Ayuningtyas Woro Hapsari Copyright (c) 2026 Sisprotek 2026-07-13 2026-07-13 5 2 ANALISIS PENGENDALIAN KUALITAS CPO PADA PROSES REFINERY GUNA MENURUNKAN FFA DI PT XYZ https://ejurnal.unival-cilegon.ac.id/index.php/sisprotek/article/view/615 <p><em>This research focuses on the analysis of the quality control system in the refining&nbsp; process of Crude Palm Oil (CPO) at PT XYZ, with the aim of minimizing the level of Free Fatty Acid (FFA) in Refined, Bleached, and Deodorized Palm Oil (RBDPO) products. The quantitative approach applied is based on the Six Sigma methodology&nbsp; through the DMAIC cycle (Define, Measure, Analyze, Improve, Control). The evaluation of the process is strengthened by triangulation of data sourced from field observations, structured interviews, internal documentation, and the company's historical track record. In the Define phase, process mapping is carried out through SIPOC diagrams and Critical to Quality (CTQ) determination. The Measure phase&nbsp; involves measuring stability through the P control map as well as calculating the values of DPU, DPO, DPMO, and sigma capabilities. Next, the Analyze phase&nbsp; utilizes Pareto and Fishbone diagrams&nbsp; to identify the root cause of the problem. Based on the findings, the average out-of-spec FFA product throughout 2025 will reach 2.9%, with a DPMO value of 9,752.3232 and a performance level of 3.87 sigma (equivalent to the national industry average). The main triggering factors for this failure come from human, machine, method, material, environmental, and measurement aspects. As a solution, in the Improve stage, recommendations based on 5W+1H were prepared which included tightening raw material supervision, maintenance of vacuum and steam spurging systems, updating SOPs, periodic instrument calibration, and operator training. Finally, the Control stage&nbsp; is designed to ensure the consistency and stability of the quality of RBDPO at PT. An sub-optimal layout can lead to backtracking, cross-movement, and extended patient flows, which negatively impact service effectiveness. This study utilizes the Systematic Layout Planning (SLP) method. A descriptive quantitative method with a case study approach was employed. Data collection was conducted using a From-To Chart, Activity Relationship Chart (ARC), and Activity Relationship Diagram (ARD). The results indicated approximately 360 movements per day. Based on this analysis, a proposed layout was designed to establish a one-way flow. The proposed layout successfully reduces potential backtracking and cross-movement, improves the integration of service flows, and enhances space utilization efficiency compared to the existing layout. This study aims to identify potential <br>hazards and analyze the level of risk in the repair of roll mill machines at PT. ABC using the <br>Hazard Identification and Risk Assessment (HIRA) method. This method involves hazard <br>identification, risk assessment based on likelihood and severity, and the determination of <br>appropriate risk control measures. The research employs a descriptive approach with data <br>collection techniques including observation, interviews, and documentation. The results indicate <br>various potential hazards such as being caught in machinery, collisions, bruises, and respiratory <br>disorders, with risk levels ranging from low to high. Therefore, risk control efforts are required <br>through the use of Personal Protective Equipment (PPE), improved safety training, and the <br>implementation of proper work procedures to minimize workplace accident risks. with a total of 11 <br>accidents, 6 minor, 4 moderate, and 1 serious. and after the research there were 11 minor <br>accidents.</p> Muhammad Azis Adhim Heri Setiawan Copyright (c) 2026 Sisprotek 2026-07-13 2026-07-13 5 2 ANALISIS TINGKAT RISIKO ERGONOMI PADA MAHASISWA TEKNIK INDUSTRI DI LABORATORIUM KOMPUTER DENGAN MENGGUNAKAN NORDIC BODY MAP DAN METODE RULA https://ejurnal.unival-cilegon.ac.id/index.php/sisprotek/article/view/617 <h4>Industrial Engineering students in the computer laboratory are at risk of experiencing ergonomic problems due to prolonged static sitting postures and workstations that do not comply with ergonomic principles. This study aims to analyze the level of ergonomic risk using the Nordic Body Map (NBM) and Rapid Upper Limb Assessment (RULA) methods and to formulate ergonomic workstation recommendations in the computer laboratory of Universitas Al-Khairiyah Cilegon. This research employed a descriptive quantitative approach involving 34 respondents for the identification of musculoskeletal disorders (MSDs) complaints using NBM and 6 selected participants for detailed posture assessment using RULA. The RULA results showed an average Grand Score of 5.17 with Action Level 3, indicating moderate to high risk that requires immediate corrective action. The main risk factors were non-neutral neck and trunk postures caused by inappropriate chair, desk, and monitor dimensions. The NBM results indicated that the highest complaints were reported in the lower back, buttocks, upper back, and upper neck, with the average complaint level categorized as “Moderately Pain.” Recommended improvements include the use of adjustable chairs with lumbar support, adjustment of desk dimensions, proper monitor and keyboard positioning, and the implementation of micro-stretching exercises and the 20-20-20 rule.</h4> Dina Rosdiana Atia Dwi Prasasti Puput Setianah Copyright (c) 2026 Sisprotek 2026-07-13 2026-07-13 5 2
